– A TRUST CORPORATION (BPI WEALTH). ALL FUNDS/PRODUCTS MANAGED BY BPI
WEALTH ARE TRUST AND/OR INVESTMENT MANAGEMENT FUNDS. THESE ARE NOT DEPOSIT
PRODUCTS AND ARE NOT AN OBLIGATION OF, GUARANTEED OR INSURED BY, BPI WEALTH, BANK OF THE PHILIPPINE
ISLANDS, ITS AFFILIATES OR SUBSIDIARIES, OR THE PHILIPPINE DEPOSIT INSURANCE CORPORATION.

This Account Opening Form represents the discussion and explanation of all pertinent documents, including the Participating Trust Agreement and the Risk
Disclosure Statement. This also confirms that an advice has been given for the investor to read all the documents related to the investment, all of which are
available online or on print upon request. The investor has also been encouraged to ask questions on matters contained in the foregoing documents and that the
same has been fully explained prior to making the investment.

Paperless Reporting and Correspondences
For BPI Online-registered clients, the Investment Account is automatically enrolled in BPI Online which provides 24/7 access to account details.
A Transaction Advice for each subscription or redemption as well as periodic Statements of Account will be sent to the registered email address of the Trustor or are otherwise
accessible through BPI Online. Printed copies shall be provided only upon request and may be subject to fees. Joint “AND” accounts cannot be enrolled in BPI Online. Joint “OR” accounts
are viewable by all the Trustors and either of the Trustors may process select transactions through their respective BPI Online accounts.
All correspondences with the Trustor shall be sent to the Primary Trustor’s address and contact information in the Customer Information Sheet or the latest available records of the
Settlement Account Bank, unless a different address or contact information is nominated through a separate letter of instruction. To the extent possible, all correspondences shall be sent
electronically through the said contact information. For inquiries, the Trustor may reach out to their branch of account.
